Case Management

Case management is collaborative, client-driven process to support people impacted by homelessness and ensure they receive access to services and supports. Client-centered case management ensures that the person who has experienced homelessness has a voice and choice on what supports they need. Client choice empowers the individual person seeking services and promotes an improved quality of life. Successful case management includes components such as

  • Intake
  • Needs assessment
  • Service planning
  • Monitoring and evaluation

At the heart of case management are principles to  make support more impactful, such as building trust, empowering clients and using trauma informed care approach that incorporates awareness of trauma at every step of service delivery.
