Coordinated Entry for Hospital Patients

Coordinated Entry is a streamlined system that assesses households using an objective tool that identifies vulnerability and barriers to housing. Those who are assessed as having the highest vulnerability and housing barriers will be prioritized for access to available housing programs as vacancies occur. Within the Atlanta Continuum of Care, there are stationary access points where clients can get assess for coordinated entry on a first come, first serve basis. Mobile assessments are performed by local PATH teams. Each client that is seen by a PATH worker or at stationary access points are assessed using the VI-SPDAT.

Patients completing a Coordinated Entry Assessment is not meant to be an immediate housing solution. Please contact Brianna Flowers at bflowers@partnersforhome.org with any questions.

Is the patient you are working with, connected to a Homeless Service Provider?

YES
Connect with the patient's homeless service system case manager.
Ask the patient if they have a case manager with an agency like Intown Cares or Gateway. If they don’t know, contact bflowers@partnersforhome.org. Connecting to the client’s case manager can play a pivotal part in the patient’s recovery process and their discharge destination. The homeless response system case manager can provide the hospital staff with confidence that the patient has access to supportive services.

Have the patient have a Coordinated Entry Assessment completed.
If the patient is between the ages of 18-24, link them to CHGA or CHRIS 180. For patients 25 and over, link to Gateway, Safe House or First Presbyterian. And for patients who are non-mobile, dial 311.

Ask patient if they are working with a homeless service provider. Ex. Gateway Center, Intown Cares.
Connect with the service provider to assist with verifying if the patient has an open CES assessment. If you need assistance identifying contact information for a homeless service provider or don't know the case manager, please contact Brianna Flowers at bflowers@partnersforhome.org
